  • Does Medicare cover home health?
    Yes, home healthcare is 100% covered by Medicare.
  • What does "homebound" mean?
    If you leave the home, absences must be infrequent or for periods of short duration. You may also leave to receive medical treatment, an occasional outing must require the assistance of ambulatory devices, other persons or special transportation. It must be a taxing effort based on their health condition. Homebound does not mean you can’t leave home! ​ According to CMS the guidelines are as follows: A person may leave home for medical treatment or short, infrequent absences for non-medical reasons, such as attending religious services, etc... Please consider the services your patient can benefit from in the comfort of their own home and become proactive before it reaches the hospitalization stage.
  • How often will the nurse or therapist come see me?
    The frequency of visits will be based on your individual health care needs. This is why we will do a complete assessment of your physical, mental, emotional, and social needs. Additionally, because our team values your input in establishing your individualized plan of care, we will ask you what your goals are for home health.
  • How does service begin?
    The initial order for home health services must come from your physician. He/she will authorize a coordinated treatment plan, also known as a "plan of care," as well as, review the services and your progress from time to time.
  • Why should I choose Home Healthcare?
    Home healthcare is meant to provide treatments for patients who have been discharged from the hospital but have yet to fully recover. This service helps prevent or postpone hospital or nursing home care. It also provides professionally supervised programs, as well as, allows for dignity, independence, and family support. On top of that, home healthcare involves no co-pay or out of pocket expense to you.
  • How is hospice care paid for?
    Hospice is a benefit paid for my Medicaid, Medicare, and many private insurance companies.
  • What is hospice?
    Faith Hospice is a service, not a place, for terminally ill patient. We serve patients and their families by addressing the physical, psychological, social, and spiritual aspects of end-of-life care.
  • What services are offered?
    Each patient is served by our team that includes: doctors, RN case managers, home health aides, social workers, chaplains, and volunteers Faith Hospice is open 7 days a week, 24 hours a day.
  • Who is eligible for hospice?
    Anyone with a 6-month or less prognosis as determined by a physician is eligible for hospice care.
  • How do I sign-up for hospice services?
